For Patients with Dental Insurance
Our office is happy to coordinate with families who are covered by dental insurance. We ask that you read your policy to be sure that you are fully aware of any benefits and limitations of your plan.
Our customary procedure in handling our patients’ accounts is the same for patients with or without insurance coverage. An initial payment is due when treatment starts. The remaining balance is paid in equal monthly installments at 0% interest. To avoid late fee, your monthly payments must be paid by the first of the month whether or not you are scheduled for a monthly adjustment.
Fee for Diagnostic Records
The fee for Diagnostic records, which includes a Panoramic and Cephalometric x-ray, Intra and Extra oral photographs, Mounted study models on an Articulator (artificial jaw simulator),and Occlusal analysis(bite study), is $535 and is due at the time of service minus the estimated insurance benefit. The information collected will be studied by Dr. Ma and a treatment plan will be formulated and a consultation scheduled.
Treatment Fee
The treatment fee covers the cost of appliances and services performed in the office during active treatment, followed by an additional two years of retention check. There will be additional charges for appliances lost or excessive breakage due to negligence. The fee does not include and general dentistry, extractions, of TADs (temporary anchorage device) needed during or related to treatment performed outside this office.
